June 30, 2024 @11:59 pm: Provider Interest Forms due for 2024 Cohort 4.
July 8th: Providers notified of selection status (accepted/waitlisted/etc.). If spaces remain, interest forms will be reviewed on a rolling basis and filled as space/capacity allows.
July 22nd - July 31st: Provider Onboarding. Program Profiles set up and provider trainings complete.
July 31st: New provider enrollments open, exact timing is based on onboarding completion (i.e. if provider is onboarded sooner, they may receive family enrollments as soon as they are up and running).
Provider Selection
If selected, providers will need to complete the following by April 2024:
Read and sign a Memorandum of Understanding (MOU)
Complete Program Profile training (1 hour)
Set up a program profile (1-3 hours)
Complete the enrollment system training (2-hour commitment)
